Once a year, groups must submit a new updated Group Information Form. The Group Information Forms will be mailed to current trip coordinators in December of each year. After the form is completed it must be mailed to: Harris County Precinct 4, SAP Transportation Office, 1731 Hugh Road Houston, Texas 77067.
Each group will be assigned a “group number,” which is determined by the order in which the completed Group Information Forms are received. Group numbers determine the order in which bus reservation phone calls are made.
The Senior Adult Program Transportation Office will contact the primary trip coordinators for each group two (2) times a year to make bus reservations. At the time the reservations are made, the destination does not have to be planned or known. Bus reservations may be scheduled up to six (6) months in advance. No reservations can be accepted with less than 30 days notice.

After each call to reserve buses, the trip coordinator will receive a trip confirmation number that is needed to complete the required Trip Reservation Form (TRF).
A TRF must be submitted to the Transportation Office 30 days prior to departure. If the form is not received 30 days prior to the departure date, trips will automatically be cancelled.
The TRF (itinerary) must be completed for each trip and include the specific details of the trip. TRFs can be submitted online.
A trip confirmation letter will be mailed to the trip coordinator. Please check the trip confirmation letter carefully and notify the Transportation Office at (281) 893-3726 immediately if corrections are needed.
Call the Transporation Office at (281) 893-3726 to confirm the maximum number of passengers, three to five days prior to the scheduled trip date.
